Let us handle the details: If you haven't already, please provide us with your dental insurance information so we may arrange your benefits prior to your visit. Sensitive personal information may be required in order to verify your benefits. Our online forms are completely secure, however, if you prefer then please call us directly at (510) 351-0931 with the necessary information.
Patients truly enjoy their visits with us so we ask that you give at least 48-hour notice should you need to cancel or reschedule your appointment. Missed appointments or late cancelations will be subject to a fee of $40-$75 depending on the type of treatment scheduled.

Our office is conveniently located in downtown San Leandro, between E. 14th St and Bancroft Ave. We are accessible via the Davis St. Exit on 880, Estudillo Ave. Exit on 580, and a 10-minute walk from San Leandro BART Station.

We have a parking lot located in the back of the building, accessible via Joaquin Ave. There will be a white sign reading "433 Estudillo Visitor Parking" where you will go UP the ramp.
